Comments:
An unannounced, on-site 60-day monitoring inspection was initiated and completed on August 28, 2024, as a part of the provisional licensure period. The on-site inspection began at 4:45 pm and ended at 5:55 pm. The inspector reviewed compliance in the areas listed above. There were 93 children present and 5 staff. The inspector reviewed 9 children?s records onsite. 5 staff records were reviewed electronically on August 30, 2024. This inspection included interviews, observations, and document review.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on September 9, 2024. A POC submitted after this date will not appear on the public website.

Violations:
Standard #: 8VAC20-770-60-C-2
Description: (Repeat Violation) Based on record review, the center failed to obtain the findings from a search of the central registry within 30 days of employment.

Evidence:
1. The record for staff # 2 did not contain a central registry background. Staff # 2?s date of hire was 7/19/2023.
2. The record for staff # 5 did not contain a central registry background. Staff # 5?s date of hire was 1/6/2010.

Plan of Correction: Central registry's are being redone. We are implementing a new system that will notify staff when required documentations are expiring.

Standard #: 8VAC20-780-60-A
Description: (Repeat Violation) Based on record review, the center did not ensure that each child shall maintain and keep at the center a separate record for each child enrolled which shall contained the required information.

Evidence:
1. The record for child # 3 did not contain the phone number of child's physician. Child # 3?s enrollment date was 8/19/2024.
2. The record for child # 5 did not contain a work number for dad. Child # 5?s enrollment date was 8/19/2024.
3. The record for child # 8 did not contain a work number for mom or dad. Child # 8?s enrollment date was 8/19/2024.

Plan of Correction: Going forward the center will ensure that each child's file will consist of completed documentation including the physicians phone number, and work phone numbers for both mom and dad.
