A subsidy health and safety (SHSI) inspection were initiated on 08/22/2024 and concluded on 08/22/2024. There were 40 children present with 13 staff supervising ranging in age from 3 months to 4 years 11 months. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.
Information gathered during the inspection determined non-compliance with subsidy regulations and violations were documented on the violation notice issued to the facility.

Standard #: 8VAC20-790-600-F Description: Based on a review of records, the vendor did not ensure that each staff complete the Virginia Annual Health and Safety Update training for 2023.
Evidence:
1. Staff 3 (DOH: 07/01/2022) contained documentation of the 3-hour annual health and safety update training 05/30/2022 and the next one was completed on 06/04/2024.
2. Staff 3 confirmed that administration did not have any staff complete the 3-hour annual health and safety training updates in the year of 2023.Plan of Correction: Once the error was noticed, all staff completed 2024 annual training.
