An unannounced monitoring inspection was initiated on 7/30/2024 and completed on 9/5/2024 in response to a self-report received by the licensing office on 7/22/2024 relating to supervision. The inspector reviewed compliance in the areas listed above. The inspector reviewed 1 child record and 4 staff records on-site on 7/30/2024. This inspection included document review, a tour of the facility and surrounding buildings and roads, and interviews. Additional interviews were conducted on 8/14 and 8/15 in person and via telephone. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 9/12/2024. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-340-A Description: Based on observations, interviews, and documentation review, the facility failed to ensure the care, protection, and guidance of children in care.
Evidence: At approximately 8:00am on 7/22/2024 child #1, age 8, became agitated when asked to write sentences as a consequence of running in the classroom. Child #1 proceeded to walk out of the classroom, and stand near the front door of the center while staff tried to calm them down. Child #1 then walked out of the building, ran across a road (with a speed limit of 25mph,) and climbed up on top of a vehicle in a parking lot approximately 150 feet away, before staff were able to catch up and secure the child. Staff brought child #1 back to the center, and they remained outside for approximately 30-45 minutes while different staff attempted to calm child #1 down and deescalate the situation. Child #1 was reportedly hitting, spitting, kicking, growling and breathing heavy. Child #1 appeared to have calmed down, but then was able to bolt from the center and staff a second time, this time running approximately 450 feet towards and onto a road with a 40mph speed limit. Staff chased child #1 until they caught up and found child #1 standing in the middle of the five-lane undivided road.Plan of Correction: Retrain staff on age-appropriate behavior guidance.
Standard #: 8VAC20-780-340-D Description: Based on interviews and documentation review, the facility failed to ensure that in each grouping of children at least one staff member who meets the qualifications of program director is regularly present.
Evidence: On 7/22/2024 there were no staff scheduled to work in the school age classroom who had program leader qualifications. The records for staff #1, staff #2, staff #3 and staff #4, who all reportedly worked in the school age classroom on 7/22/24, did not have documentation of group leader qualifications.Plan of Correction: Seek and schedule training to ensure my staff stays current on their lead qualification skills.
Standard #: 8VAC20-780-400-B Description: Based on interviews and documentation review, the facility failed to ensure all behavioral guidance was constructive in nature, age and stage appropriate, and intended to redirect children to appropriate behavior and resolve conflicts. Evidence: Between approximately 7:30-8:00am on 7/22/2024, two children in the school age classroom were running around, telling other kids to ?shut up.? The two children were asked to follow classroom rules, and warned that if they didn?t they would have to sit and write sentences. After a final warning the two children were asked to sit and write sentences, ?I will not run in class.? Child #1 became extremely agitated, refused to write sentences, and walked out of the classroom. Plan of Correction: Retrain staff on age-appropriate behavior guidance.
