An unannounced inspection was initiated and completed on 08/27/2024 in response to a complaint received by the licensing office on 08/23/2024 relating to allegations of emergency procedures and operational responsibilities. The inspector reviewed one children?s record and two staff records on-site. This inspection included:

? tour of the facility
? interviews
? observations
The preponderance of evidence gathered during the investigation supports the allegations; therefore, the complaint is determined to be valid. Information gathered during the inspection determined noncompliance with applicable standards or law and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 09/13/2024. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-40-E Complaint related: Yes Description: Based on interviews and documentation review it was determined that the facility did not always maintain compliance with their own policies and procedures. Evidence: The facilities safety manual states that immediate application of ice in a plastic bag or a frozen sponge in a plastic bag will be applied to any bumps or bruises sustained at the facility. In the older infant room on 08/22/2024 at 9:20 am, child A (DOB: 09/27/2023) sustained two bumps to the back of their head. It was reported that staff briefly held a cold water bottle to the child's head and management was notified about the injury. Plan of Correction: On 8/28/24, teachers and management reviewed the injury prevention plan and safety manual. Proof of injury prevention signed by all teachers and management.
Standard #: 8VAC20-780-40-I Complaint related: No Description: Based on interviews it was determined that the facility did not have written procedures for injury prevention. Evidence: The center's injury prevention procedures were not accessible to the inspector during the inspection. Plan of Correction: An updated injury prevention plan has been established and distributed throughout the center. Staff members have signed a copy of the prevention plan on 9/11/24, proof of injury prevention signed by all teachers and management.
Standard #: 8VAC20-780-80-B-2 Complaint related: Yes Description: Based on interviews and documentation review it was determined that the provider failed to notify the department within two business days when a child under their supervision sustains an injury requiring outside medical attention. Evidence: It was reported that on 06/28/2024 child A (DOB: 09/27/2023) sustained an injury to their forehead requiring medical treatment. The department was not notified regarding this injury. Plan of Correction: On 8/28/24, teachers and management reviewed the injury prevention plan and safety manual. Moving forward, management will properly document serious injuries. Proof of injury prevention signed by all teachers and management.
Standard #: 8VAC20-780-550-N Complaint related: Yes Description: Based on interviews and documentation review it was determined that the facility failed to notify parents immediately when a child sustains a serious injury. Evidence: In the older Infant room on 08/22/2024 at approximately 9:20 am Child A (DOB: 09/27/2023) sustained an injury to the back of their head while attempting to pull themselves up by a chair. The child was observed to have two bumps on the back of their head. The parent of child A was not notified of the incident until 3:04 pm on 08/22/2024 via telephone call. Plan of Correction: On 8/28/24, management reviewed policy and procedures in the family handbook, safety manual, and the injury prevention plan was updated. Proof of injury prevention signed by all teachers and management.
