An unannounced, on-site monitoring inspection was initiated and completed on 09/09/2024. The on-site inspection began at 10:20 AM and ended at 12 PM. The inspector reviewed compliance in the areas listed above. There were 16 children present and 6 staff. The inspector reviewed 5 children?s records and 2 staff records onsite. This inspection included: document review, tour of the facility, interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 8VAC20-780-60-A Description: Based on record review, the center failed to ensure that the separate record for each
enrolled child shall contain all the elements as required by the standards.
Evidence:
The record for Child #4 did not include the following information: address for one of the
two designated people to call in an emergency if a parent cannot be reached.Plan of Correction: The director will obtain the missing information from the parent and will monitor.
