An unannounced monitoring inspection was initiated on June 17, 2024 and completed on September 13, 2024 in response to a self-report received by the licensing office on June 11, 2024 relating to forbidden actions. The inspector reviewed one child?s records and three staff records on-site.
This inspection included document review, tour of the facility, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on September 24, 2024. A POC submitted after this date will not appear on the public website.
The Violation Notice was corrected to identify the violation as a Repeat violation on October 7, 2024.

Standard #: 8VAC20-780-410-1 Description: REPEAT VIOLATION
Based on staff interviews and review of documentation, the center failed to ensure that staff did not engage in forbidden actions of rough handling when supervising children.
Evidence:
On June 11, 2024:
1. Staff #1 roughly handled Child #2, age 2.5 years, by pushing Child #2 in the forehead with Staff #1?s hand. Child #2 cried and pointed at Staff #2. Child #3 told Staff #3 ?teacher pushed forehead?
2. Staff #1 roughly handled Child #2, age 2.5 years, by grabbing Child #2 by the wrist and jerking Child #2?s wrist back. Child #2 cried.Plan of Correction: Program policies, training and
monitoring systems will continue to
support the prevention of Forbidden
Actions. To ensure behavioral
guidance is constructive in nature,
ages and stages appropriate, and will
support the redirection of children to
appropriate behavioral to assist conflict
being resolved. The definition of
Forbidden Action as well as examples
given will be transparently shared.
Program will continue to provide
training at the time of hiring as well as
offering during our annual preservice
training; we will very transparent and
use this situation as an example of how
active supervision, behavioral guidance
will be constructive in nature while age
appropriate supporting redirection and
how to assist children in resolving
conflicts.
