An unannounced, on-site renewal inspection was initiated on 9/18/2024 and completed on 9/18/2024. The on-site inspection began at 10:15am and ended at 11:45am. The inspector reviewed compliance in the areas listed above. There were 12 children present and 4 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 9/26/2024. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-60-A Description: Children's records are required to contain parental employment phone numbers, documentation of child updates and confirmation of up-to-date information.
The record for child #5 was missing the employment phone number for both parents. The child was present during the inspection and had been in care for 3 months. The record for child #2 and child #3 did not contain documentation of an annual update to the child?s record or information. Child #2 and child #3 have both been in care for over 2 years.Plan of Correction: The director will make sure to contain parental employment phone number on each child records as required by standards. The director will add copy to all children records documentation of annual update.
