Standard #: 8VAC20-780-70 Description: Based on record review and staff interviews, the licensee did not ensure there was documentation to demonstrate an individual possesses the education, certification, and experience required by the job position.
Evidence: Staff #2 designated Program Lead does not have qualifying documentation.Plan of Correction: Staff member requested transcripts. Transcripts will be added to file.
Standard #: 8VAC20-780-280-B Description: Based on observation and staff interviews, the licensee did not ensure hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.
Evidence: Disinfectant wipes were observed in an unlocked cabinet in near the restrooms and hand sanitizer was observed on a shelf in the 3?s room. These items were labeled "keep out of reach of children".Plan of Correction: Wipes cabinet has a new safety lock added. Hand sanitizer moved to a locked cabinet.
