An investigation was initiated on 07/25/2024 and completed on 09/19/2024 in response to an Injury Report of Outside Medical Treatment submitted by the center and received by the licensing office on 07/25/2024. This investigation included: document review, tour of the facility, interviews and observations. The preponderance of evidence gathered during the investigation does not support a violation(s) with applicable standards or law. No violations were issued.
