Comments:
An unannounced, on-site renewal inspection was initiated and completed on 9/23/2024, as a part of the conditional licensure period. The on-site inspection began at 9:50am and ended at 12:15pm. The inspector reviewed compliance in the areas listed above. There were 38 children present and 7 staff. The inspector reviewed (3) children?s records and (10) staff records on-site on 9/23/2024. This inspection included document review, emergency drill logs, medication authorization forms, allergy action forms, tour of the facility and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on (9/30/2024). A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-60-A
Description: Repeat Violation
Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information.
Evidence:
1.The record for Child #2, enrolled for two years five months, was missing the following required components: both parent's work phone numbers and place of employment for one
parent.
2. The record for Child #3, enrolled one day, was missing the following required components:

Description: Staff are to complete the VDOE sponsored online orientation course within 90 days of hire. Evidence:
1. Staff #7 had not completed the course and is now two months past due.

Description: Any child for whom emergency medications (such as albuterol, glucagon, and
epinephrine auto injector) have been prescribed shall always be in the care of a
staff member or independent contractor who meets the requirements in
subdivision 1 of this subsection.
Evidence:
1. Child #2 who has been prescribed emergency medications was not always in the care of a staff member who meets the requirements in subdivision 1 of this subsection on 9/23/2024 from approximately 9:28am to 10:15am..

Description: The center shall provide to the licensing representative an annual fire inspection
report from the appropriate fire official having jurisdiction.

Standard #: 8VAC20-780-270-A
Description: Repeat Violation
The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition.

1. In the infant classroom, the safety lock is broken to keep the cabinet locked.
2. In the Discovery Pre 1 classroom, the wall carpet is peeling off approximately 4 inches. The children's sink has rough edges/splintering on the bottom of the cabinet on two sides.
3. The poured in place on the Toddler playground is sunken in creating a tripping hazard approximately 12 inches by 12 inches.

Plan of Correction: 1. Cabinet safety lock has been replaced for temporary fix.Permanent fix work order has been placed

Description: Repeat Violation
Hazardous substances must be in a locked place that prevents access by children.
Evidence:
1. In the infant classroom a container of Clorox wipes and Clorox Water spray were inside an unlocked cabinet near the changing table. The lock was broke.
2. In the school age classroom, Lysol spray disinfectant, Fabulosa, Glass Cleaner, two Deep Clean Neutralize bottles, 9 large hand sanitizers, Round Up weed and grass killer/fertilizer and bleach were on shelves in an unlocked closet.
3. In the Storage Room/Teacher room a spray bottle of Clorox water was on top of the counter.

Plan of Correction: I. Cabinet safety lock has been replaced for temporary fix. Permanent fix work order has been placed.
2. Teachers have been instructed keep all cleaning products in locked cabinets, located in classroom bathroom.
3. Cabinet in classroom to be used for bleach water - lock installed

Standard #: 8VAC20-780-320-B
Description: Each restroom area provided for children shall be equipped with soap, toilet paper, and disposable towels or an air dryer within reach of children.
Evidence:
1. In the Discover Pre 1 classroom, the bathroom did not have toilet paper. Staff stated the children do not use the bathroom.
2. In the Preschool classroom, the bathroom did not have toilet paper. Staff stated it was kept in the cabinet.
3. In the School Age classroom, the bathroom did not have paper towels.

Standard #: 8VAC20-780-500-A
Description: Repeat Violation
Children?s hands shall be washed with soap and running water after toileting, or any contact with blood, feces or urine.
Evidence:
A staff person in the Discover Pre 1 classroom changed a child?s diaper and did not wash the child's hands with soap and water.

Plan of Correction: Teachers are receiving individual copies of "Help Us Prevent the Spread of Germs" a guide on when handwashing should occur

Standard #: 8VAC20-780-520-C
Description: Repeat Violation
If diaper ointment or cream is used, the following requirements shall be met:
These products do not need to be kept locked but shall be inaccessible to children.
Evidence:
1. In the Discovery Pre 1 classroom, a diaper ointment was under the changing table cabinet and was accessible to children.

Plan of Correction: Teacher has been instructed to keep the ointments out of children's reach by locking changing table cabinets
