Comments:
An unannounced inspection was initiated on June 17, 2024 and concluded on (date) in response to a complaint received by the licensing office on June 20, 2024 related to allegations of supervision. The inspection included document review, tour of the facility, interviews, and observations
The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on (10/30/2024). A POC submitted after this date will not appear on the public website.

Description: The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are in compliance with the center's own policies and procedures that are required by these standards. In the center?s staff handbook, it states "children shall be in the sight and sound of the employees at all times". Children were out of sight supervision of staff while in the playhouse on the playground. There was inappropriate contact at that time.

Description: When staff are supervising children, they shall always ensure their care, protection, and guidance. On 6/11/2024, Child 1 (age 7) and Child 2 (age 8), were in a playhouse on the playground with the windows blocked with rubber puzzle mats so they could not be seen. A third child witnessed the two children?s hands down each other?s pants in the playhouse. The children reportedly admitted to Staff 2 and Staff 3 that there was inappropriate touch during this time.
The week prior, Staff 2 observed Child 1 and Child 2 sharing a blanket during rest time. The children admitted to Staff 2 that there was inappropriate touch during this time.

Description: Children under 10 years of age always shall be within actual sight and sound supervision of staff. On 6/11/2024, two children (age 7 and age 8) blocked the windows of the playground playhouse and could not be seen or heard by staff. The week prior, the two children were under a blanket together. The children were engaged in inappropriate touch while out of sight supervision.
