Technical Assistance:
1. Water lead testing results are to be submitted to the Virginia Department of Health, Office of Drinking Water.
2. Recommend that emergency evacuation plan include a location that provides shelter during inclement weather.
3. The inspector will send information about obtaining an Environmental Health Inspection and staff training.

Comments:
An unannounced, on-site monitoring inspection was conducted on 9/26/24. The on-site inspection began at 10:55am and ended at 12:30pm.The inspector reviewed compliance in the areas listed above. There were (21 children present and 4 direct-care staff. The inspector reviewed 2 children?s records and 5 staff records while on-site. This inspection included document review, tour of the facility and interview. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-550-P
Description: The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence. The record shall include (but not limited to) documentation of how the parent was notified, the date and time the parent was notified and 2 signatures.

1 of 7 written injury reports did not include documentation of how the parent was notified, or the date/time the parents was notified; while another report did not include 2 signatures.

Plan of Correction: The written injury reports were corrected during the inspection.
