During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced, on-site renewal inspection was initiated 09/24/2024 and completed with a staff record review on 09/25/2024, as a part of the licensure period. The on-site inspection began at 3:45 PM and ended at 4:40 PM. The inspector reviewed compliance in the areas listed above. There were 25 children present and 3 staff. The inspector reviewed 5 children?s records on site and 3 staff records at the main office. This inspection included:
- document review (i.e. Injury logs, emergency drill logs, medication administration logs, authorization forms),
- tour of the facility,
- interviews, and
- observations
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/07/2024. A POC submitted after this date will not appear on the public website.
Compliance with all applicable regulations and law shall be maintained and any areas of noncompliance must be corrected.
Within 15 calendar days of your receipt of the inspection findings (inspection summary, violation notice, and supplemental information), you may request a review and discussion of these findings with the inspector's immediate supervisor. To make a request for review and discussion, you must contact the licensing supervisor at the regional licensing office that serves your geographical area. Regardless of whether a supervisory review has been requested, the results of the inspection will be posted to the DSS public website within 5 business days of your receipt of the Inspection Summary and/ or Violation Notice.
