An unannounced, on-site monitoring inspection was initiated on 09/24/2024 and completed on 09/24/2024, The on-site inspection began at 4:10 PM and ended at 4:50 PM. The inspector reviewed compliance in the areas listed above. There were 34 children present and three staff. The inspector reviewed five children?s records and two staff records on-site. This inspection included document review, tour of the facility, interviews and observations.
The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/08/2024. A POC submitted after this date will not appear on the public website.

Standard #: 22.1-289.035-B-1 Description: The center is required to obtain a completed sworn statement prior to the employee's first day of employment.
The sworn statement for Staff 1, who is currently working, was dated 12 days after their first day of employment.Plan of Correction: We have (1) met with administrative staff to review the hiring and onboarding process for licensed employees, (2) created a checklist with party responsible, and (3) implemented an internal audit process to review new hire files to ensure compliance.
