During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced staff record inspection was initiated and completed on 09/25/2024, as a part of the licensure period. The inspection began at 1:05 PM and ended at 1:21 PM. The inspector reviewed compliance in the areas listed above. This inspection included review of 4 staff records.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on (date). A POC submitted after this date will not appear on the public website.
Compliance with all applicable regulations and law shall be maintained and any areas of noncompliance must be corrected.
Within 15 calendar days of your receipt of the inspection findings (inspection summary, violation notice, and supplemental information), you may request a review and discussion of these findings with the inspector's immediate supervisor. To make a request for review and discussion, you must contact the licensing supervisor at the regional licensing office that serves your geographical area. Regardless of whether a supervisory review has been requested, the results of the inspection will be posted to the DSS public website within 5 business days of your receipt of the Inspection Summary and/ or Violation Notice.

Standard #: 8VAC20-780-70
Description: Staff records kept for each staff must include the verification of age and at least two references as to character and reputation obtained before employment.

the record provided for staff 1 did not include verification of age.
The record provided for staff 1, employed for 29 months, did not include documentation of two references as to character and reputation.

Plan of Correction: The facility responded with the following:
Staff member has provided a valid ID and that has been documented on the staff file as of 9/25/2024.
Two references have been obtained and documented in the staff file as of 9/26/2024.

Standard #: 8VAC20-780-240-E
Description: Within 30 days of the first day of employment, staff must complete orientation training in first aid and cardiopulmonary resuscitation.

the record provided for staff 2, employed for eight months, did not include documentation of first aid and cardiopulmonary resuscitation.

Staff member has been enrolled in a cpr course on 10/12/2024
