An unannounced, on-site monitoring inspection was initiated on 9/27/2024 and completed on 9/27/2024.The on-site inspection began at 3:30pm and ended at 5:04pm. The inspector reviewed compliance in the areas listed above. There were 17 children present and two staff. The inspector reviewed five children?s records and five staff records on-site on 9/27/2024. This inspection included document review, tour of the facility, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/9/2024. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-770-60-B Description: The center is required to obtain a completed sworn statement prior to the employee?s first day of employment.
The record for Staff #1, did not have a completed sworn statement in the file, prior to first day of employment. Staff #1 has been employed for seven months.Plan of Correction: HR has emailed over sworn statement
Standard #: 8VAC20-770-60-C-2 Description: The center is required to obtain a central registry request prior to the employment and within 30 days.
Staff #2, who is currently working with children, did not have documentation of a completed central registry request in the file. Central registry request is four months overdue.Plan of Correction: Staff, program director and HR will follow up with central registry if not received in 30 days.
Standard #: 8VAC20-780-160-A Description: REPEAT VIOLATION
Documentation of a negative tuberculosis screening shall be submitted at the time of employment and prior to coming into contact with children.
The record for Staff #1, did not have documentation of a negative tuberculosis screening in the file. Staff #1 has been employed for seven months.
Staff #2 tuberculosis screening was received 14 days after date of hire.Plan of Correction: Program director will ensure that all staff have TB completed upon hiring.

Staff #1 has been employed for seven months and Staff #2 has been employed for four months.Plan of Correction: Staff # 1 completed VDOE 10hr on March 15, VDOE 3hr March 12.

Standard #: 8VAC20-780-270-A Description: Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to, rusting of any equipment.
The handles on the drawers where the games were stored were rusted, and was accessible and in use by children.Plan of Correction: Property director notified, will instate plan to replace handles.
