An unannounced, on-site monitoring inspection was initiated on 10/2/2024 and completed on 10/202/2024. The on-site inspection began at 3:10 pm and ended at 4:00 pm. The inspector reviewed compliance in the areas listed above. There were seven children present and three staff. The inspector reviewed five children?s records and four staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 8VAC20-780-40-G Description: The center shall meet the proof of child identity and age requirements as stated in ? 22.1-289.049 of the Code of Virginia.
The was no proof of identity in Child #2's record.Plan of Correction: The Director will ensure the proof of identity is obtained and documented in the child's record.
Standard #: 8VAC20-780-90-A Description: A written agreement between the parent and the center shall be in each child's record by the first day of the child's attendance.
The record for Child #2 was missing a written agreement between the parent and the center.Plan of Correction: The Director will ensure a written agreement between the parent and the center is in the child's record.
Standard #: 8VAC20-780-540-C Description: The center is required to ensure the minimum requirements for a first aid kit.
The first aid kit was missing adhesive tape, triangular bandages, scissors and an antiseptic
cleansing solution/pad.Plan of Correction: The Director is ordering the missing items.
