Technical Assistance:
Effective January 1, 2025, the VDOE will begin determining compliance with ? 22.1-289.057 of the Code of Virginia, which is legislation passed by the General Assembly in 2020. The law requires all licensed child day programs, religious exempt child day centers that serve preschool age children, and certified preschools to test potable drinking water. The law requires that programs submit their plans and test results to the Virginia Department of Health Office of Drinking Water (VDH ODW) and the Superintendent. If the results of the testing indicate elevated lead levels, the program shall remediate, retest, and resubmit results to VDH ODW and the Superintendent. There is an additional alternative bottled water option that comes with additional requirements. Comments:
An unannounced monitoring inspection was initiated on September 3, 2024, and completed on October 8, 2024 in response to a self-report received by the licensing office on August 30, 2024 relating to Staffing and Supervision. The inspector reviewed one child?s record and two staff records electronically on October 7, 2024. This inspection included document review, tour of the facility, interviews, and observation.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on October 25, 2024. A POC submitted after this date will not appear on the public website.

Violations:
Standard #: 8VAC20-780-40-E
Description: The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with the center's own policies and procedures that are required by these standards.

On August 30th Staff #1 and Staff #2 did not follow center supervision policies which requires completing face to name checks when transitioning children from the playground back into the building and classroom. As a result of the policy not being followed Child #1 (30months old) was left unsupervised on the playground for approximately 2 ? minutes.

Plan of Correction: Staff #1 and #2 are no longer employed with the company. The new staff members assigned to the room have been trained & retrained on our face-to-name and supervision policy. The training took place on 9/11/24. Moving forward management will conduct periodic FTN observations to ensure proper processes are followed" .

Standard #: 8VAC20-780-340-A
Description: Staff will always ensure the care and protection of children when supervising them.

Child #1 is seen on video outside two classrooms attempting to gain entry by jumping up at the window, kicking a door and knocking on a door. Child #1?s face was contorted in such a way to suggest he was distressed and crying. Staff #1 stated that she was unaware the child was missing from the group.

Plan of Correction: Staff #1 and #2 are no longer employed with the company. The new staff members assigned to the room have been trained & retrained on our face-to-name and supervision policy. The training took place on 9/11/24. Moving forward management will conduct periodic FTN observations to ensure proper processes are followed.

Standard #: 8VAC20-780-340-F
Description: Staff are required to maintain actual sight and sound supervision of children under 10 years of age.

Child #1 (30 months old) was left on the playground unsupervised by any staff for approximately 2 ? minutes

Plan of Correction: Staff #1 and #2 are no longer employed with the company. The new staff members assigned to the room have been trained & retrained on our face-to-name and supervision policy. The training took place on 9/11/24. Moving forward management will conduct periodic FTN observations to ensure proper processes are followed.
