The play ground equipment is not currently in use by children due to graffiti on the equipment. Although the children are not currently using the equipment, please ensure that the area is cleaned daily and checked for hazards.

An unannounced monitoring inspection was completed on 10/09/2024 in response to a self-report received by the licensing office on 09/16/2024 relating to Programs. A mandated inspection was also completed as part of the licensure period. The inspector reviewed 6 children?s records and 7 staff records. This inspection included: document review, tour of the facility, interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self-report, violations that were not reported were found related to Programs and are documented on the violation notice issued to the program.

Standard #: 8VAC20-780-260-A Description: (REPEAT VIOLATION) The center shall provide an annual fire inspection from the appropriate fire official having jurisdiction.
The last documented fire inspection of the center was dated 11/09/2021.Plan of Correction: The director contacted administration to let them know that a fire inspection needs to be conducted at the facility. Administration will contact the fire marshal to schedule a visit and document the phone calls.
Standard #: 8VAC20-780-410-1 Description: The center is forbidden to use actions such as physical punishment and rough handling as punishment for a child's behavior.
On 09/13/2024, staff #1 used rough handling and physical punishment after child #2 bit child #1 on the arm. Proceeding the bite, Staff #1 roughly grabbed child #2 (the biter) by the arm and sat him on her lap. After child #1 refused her direction to bite child #2 back, she leaned her head forward and bit child #2 on his arm and stated, ?That?s how it feels.?Plan of Correction: Staff was immediately terminated and the incident was reported to Child Protective Services and to licensing.
Standard #: 8VAC20-780-410-3 Description: It is forbidden for a center to use punishment by another child as a form of behavioral guidance.
On 09/13/2024, child #2 bit child #1 on the arm. Video footage and interview confirmed staff #1 encouraged the bitten child #1 to punish the biter child #2 when she told him, "Bite him back! Bite him!" while holding child #2's arm to his mouth.Plan of Correction: Staff was immediately terminated and the incident was reported to Child Protective Services and to licensing.
