Comments:
An unannounced, on-site monitoring inspection was initiated on 10-9-2024 and completed on 10-10-2024. , as a part of (probation, the conditional licensure period, the provisional licensure period). The on-site inspection began at 2:40pm and ended at 5:20pm. The inspector reviewed compliance in the areas listed above.
There were 44 children present and 10 staff. The inspector reviewed 5 children?s records and 5 staff records on site. This inspection included document review, tour of the facility, interviews, observations and measurements.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10-17-2024. A POC submitted after this date will not appear on the public website.

Violations:
Standard #: 22.1-289.035-A
Description: The center is required to obtain background checks from any state in which the individual has resided in the preceding five years. Staff #2, employed for more than 4 months, did not have documentation of requesting a central registry check from one state and no documentation of requesting a central registry check or criminal history check from a different state.

Plan of Correction: The center will ensure that each state background check will show documentation of request when doing background checks.

Standard #: 8VAC20-780-140-A
Description: Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance. There was no physical examination in the record of child #1, who had been attending for 6 weeks.

Plan of Correction: The center will ensure each child record will obtain a physical examination within 30 days of attendance

Standard #: 8VAC20-780-150-B
Description: Immunization records are required to be signed and dated by a physician, his designee, or an official of the local health department. The immunization record for child #1 was not signed and dated.

Plan of Correction: The center will ensure immunizations records are signed and dated in each child's record.

Standard #: 8VAC20-780-40-M
Description: The center is required to keep a current list of all known allergies, sensitivities, and dietary restrictions in each room or area where children are present. The list must be dated and kept confidential. Child #2, has an allergy documented by a physician and it is not on a list in the room where Child #2 was during the inspection.

Plan of Correction: The center will put allergy list in play for each child that have allergies, sensitivities, dietary restrictions in each room

Standard #: 8VAC20-780-60-A
Description: Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The record for Child #1, attending 6 weeks, was missing the following required components: parents employment information, second emergency contact address, allergies/heath problems and the record for Child #3 is missing the physicians phone number.

Plan of Correction: The center will ensure each child enrolled will have parents employment information and records and pay parent emergency contact addresses, allergies, health problems and missing physician phone number filled out completely on each form

Standard #: 8VAC20-780-70
Description: Documentation to demonstrate that the individual possesses the education, certification, and experience required by the job position, and orientation and training as required shall be in each staff record. Staff #3 , designated as a Program Leaders and does not have documentation of qualifications

Plan of Correction: The center will ensure each staff have documentation and qualifications in each staff records

Standard #: 8VAC20-780-330-B
Description: (Repeat Violation)
Under equipment with moving parts or climbing apparatus, a fall zone free of hazardous obstacles must be created. The fall zones for the two meri-go-cycles and 1 spring bouncer had less than the required 6 feet of surrounding fall zone. The 2 meri-go-cycles had about 4 feet 11 inches between them and one also had about 3 feet between it and the playground boarder. The spring bouncer had about 4 feet 2 inches between it and a fence.

Plan of Correction: The center will ensure the equipment in question meet the 6 feet surrounding fall zone for each equipment to meet state standards.

Standard #: 8VAC20-780-340-D
Description: Each grouping of children shall have at least one staff regularly present who meets lead teacher qualifications. Staff #2, does not meet the lead teacher qualifications, and was designated as the lead teacher for the two?s room during the inspection.

Plan of Correction: The center will ensure each room will have a teacher that meets lead qualifications when being designated for lead teacher

Standard #: 8VAC20-780-570-E
Description: Repeat Violation
The center is required to ensure prepared infant formula shall be refrigerated, dated and labeled with the child's name. There were 6 bottles observed with no name or date and two with no date.

Plan of Correction: The center will ensure each child items are labeled each day
