An unannounced monitoring inspection was initiated on 9/19/2024 and completed on 10/2/2024 in response to a self-report received by the licensing office on 9/13/2024 relating to behavioral guidance. This inspection included - document review (i.e. Injury logs, policies and procedures, emergency drill logs, medication administration logs, authorization forms), and interviews.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 8VAC20-780-400-B Description: Behavioral guidance shall be constructive in nature and age and stage appropriate. Staff #2 placed scotch tape around both the thumbs of Child #1, 3 years old, in efforts to correct thumb sucking. Plan of Correction: Provider will train staff on appropriate ways to redirect children.
