During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Technical Assistance:
Discussed required documentation for injury reports and emergency drill records. Discussed requirements for annual updates for children's records.

Effective January 1, 2025, the VDOE will begin determining compliance with ? 22.1-289.057 of the Code of Virginia, which is legislation passed by the General Assembly in 2020. The law requires all licensed child day programs, religious exempt child day centers that serve preschool age children, and certified preschools to test potable drinking water. The law requires that programs submit their plans and test results to the Virginia Department of Health Office of Drinking Water (VDH ODW) and the Superintendent. If the results of the testing indicate elevated lead levels, the program shall remediate, retest, and resubmit results to VDH ODW and the Superintendent. There is an additional alternative bottled water option that comes with additional requirements. Comments:
An unannounced, on-site monitoring inspection was initiated on 10/15/2024 and completed on 10/15/2024, as a part of the licensure period. The on-site inspection began at 11:00am and ended at 12:20pm. The inspector reviewed compliance in the areas listed above. There were 18 children present and 5 staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included document review and a tour of the facility.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-160-C
Description: REPEAT VIOLATION
A tuberculosis (TB) screening is required every two years from the last screening.

Staff #1's updated screening was due 4 months ago and Staff #4's updated screening was due 2 months ago. The updated screenings have not yet been completed.

Plan of Correction: Will ask staff members to complete their updated TB screenings and obtain documentation for records.

Description: The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition. T

There was approximately a 1-inch section of peeling paint on the wall near the bookshelf in the Infant/Toddler classroom.

Plan of Correction: Will fix it.

Standard #: 8VAC20-780-280-B
Description: Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.

A can of Clorox disinfectant wipes with warning labels, ?Keep out of reach of children,? was found on top of the diaper changing table in the Toddler classroom, within reach of children. Bottles of Clorox, Windex and other hazardous cleaning materials were stored on the floor and low open shelfs in the unlocked staff bathroom located by the Preschool classroom and playground exit door. The staff bathroom door was unlocked and does not have a safe locking method that prevents access by children.

Plan of Correction: Will remind and retrain staff on the proper and safe storage of hazardous substances. Will remove the hazardous cleaning materials and bottles to a locked cabinet.

Standard #: 8VAC20-780-290-A-3
Description: In areas used by preschool age or younger children, electrical outlets shall have protective covers.

Three uncovered outlets were within reach to children in the Infant/Toddler room.

Plan of Correction: Corrected during the inspection. Outlet covers were placed on the outlets.

Standard #: 8VAC20-780-520-A
Description: Over-the- counter skin products shall not be kept or used beyond the expiration date of the product.

Two diaper creams were kept in the Infant/Toddler classroom beyond the expiration date of the products. Child #2's diaper cream expired 1.5 months ago. Child #4's diaper cream expired 3.5 months ago.

Plan of Correction: Will remove and return the expired diaper creams back to the parents.

Standard #: 8VAC20-780-520-C
Description: Diaper ointment or creams products must be stored inaccessible to children.

An open container with diaper creams was stored on the bottom, low shelf beneath the diaper changing table, making it accessible to children in the Infant/Toddler classroom.

Plan of Correction: Will remind and retrain staff on the proper and safe storage of diaper creams.

Standard #: 8VAC20-780-550-D
Description: The center is required to implement a monthly practice of an evacuation drill.

The center did not practice a monthly evacuation drill for the month of September. The last drill was practiced in August 2024.

Plan of Correction: Will remember to schedule and complete monthly evacuation drills.
