Effective January 1, 2025, VDOE will begin determining compliance with ? 22.1-289.057 of the Code of Virginia, which is legislation passed by the General Assembly in 2020; Background checks for all staff counted in direct sight and sound supervision of children in care, daily health observation training.

An unannounced, on-site monitoring inspection was initiated on October 15, 2024 and completed on October 15, 2024, as a part of licensure period. The on-site inspection began at 10:42 am and ended at 1:13 am. The inspector reviewed compliance in the areas listed above.
There were 14 children present and 4 staff, and 1 administrative staff. The inspector reviewed 5 children?s records and 3 staff records on-site. This inspection included document review.
,tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on November 7, 2024 . A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-140-A Description: Physical examination completed by a physician shall be obtained prior to the first day of attendance within 30 days after the first day of attendance.
There was no physical examination in the record of Child#2, who had been enrolled for 2 months.Plan of Correction: Initially a physical was received however illegible-parents contacted and legible physical obtained
Standard #: 8VAC20-780-70 Description: Staff records shall be kept for each staff person.
The record for Staff#3 was not available for review. The staff was present and supervising children in care during the inspection.Plan of Correction: started a file for staff (volunteer)
Standard #: 8VAC20-780-280-B Description: Hazardous substances shall be kept in a locked place using a safe locking method that prevents access by children.
A container of cornstarch baby powder with the warning labels, "keep out of reach of children, were unlocked on top of the cubby , within reach of the children.Plan of Correction: baby power has been placed within a locked place
Standard #: 8VAC20-780-350-B-1 Description: The required staff-to-children ratios for birth to 16 months is one staff per four children. Staff#1 and Staff#2 were observed supervising 10 children (youngest child 13 month old) from approximately 10:42 am -11:30 am. Plan of Correction: staff to children ration has been adjusted to suit the youngest child in the toddler room

wipes before and after eating meals or snacks.
The children's hands in the toddler group were not washed with soap and running water before eating lunch.Plan of Correction: Staff were advised to follow the daily schedule every day faithfully (written schedule located on the toddler wall) which include bathrooms & to wash hands before lunch
