Comments:
A virtual Subsidy Health and Safety inspection was conducted on 8/14/2020. Due to the COVID pandemic the inspector was not on site and the inspection was conducted via mobile video application. There were 2 children present during the inspection, observed sleeping on separate cots. The home, children's files and vendor documentation was reviewed. Emergency and operating equipment and procedures were also inspected. Violations observed are indicated in the violation notice.

Description: Based on virtual review of child records it was determined that all required information was not maintained.

Evidence: During inspection Child1, 2, 3, and 4 were missing: Parent's work information, Emergency Contact information and Proof of identification.
