During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Effective January 1, 2025, the VDOE will begin determining compliance with ? 22.1-289.057 of the Code of Virginia, which is legislation passed by the General Assembly in 2020. The law requires all licensed child day programs, religious exempt child day centers that serve preschool age children, and certified preschools to test potable drinking water. The law requires that programs submit their plans and test results to the Virginia Department of Health Office of Drinking Water (VDH ODW) and the Superintendent. If the results of the testing indicate elevated lead levels, the program shall remediate, retest, and resubmit results to VDH ODW and the Superintendent. There is an additional alternative bottled water option that comes with additional requirements. An unannounced, on-site renewal inspection was initiated on 10/22/2024 and completed on 10/22/2024. The on-site inspection began at 2:00pm and ended at 4:00pm. The inspector reviewed compliance in the areas listed above. There were 14 children present and 4 staff. The inspector reviewed 5 children?s records and 6 applicant/agent/staff records on site on 10/22/2024. This inspection included document review, a tour of the facility, observations and interviews. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/1/2024. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-60-A Description: (Repeat Violation) Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information.
The record for Child #1, enrolled for 6 months, was missing proof of birth and the name, address and phone number for a second emergency contact. The records for Child #2, #3, #4, #5 and Child #6 were missing the date of enrollment.Plan of Correction: Child #1, the person that reviewed the birth certificate signed;
Standard #: 8VAC20-780-60-A-8 Description: (Repeat Violation) A written care plan from a physician is required for each child with a diagnosed food allergy. There was no allergy care plan on file for child #1, enrolled for 6 months, who has a shellfish allergy. Plan of Correction: Gave parents until 11/20/2024 to submit allergy care plan to avoid suspension of child.
Standard #: 8VAC20-780-80-A Description: For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.
There was no attendance record for the group of five children in the multipurpose room.Plan of Correction: Staff was informed to make sure they are keeping a log.
Standard #: 8VAC20-780-240-A Description: Staff are to complete the VDOE sponsored online orientation course within 90 days of hire.
Staff #2, employed for 6 months, Staff #3, employed for 7 months, Staff #4, employed for 6 months, Staff #5, employed for 4 months and Staff #6, employed for 4 months, have not completed the course.Plan of Correction: All staff members have signed up to complete the course.
Standard #: 8VAC20-780-340-D Description: (Repeat Violation) Each grouping of children shall have at least one staff regularly present who meets lead teacher qualifications.
Staff #3, Staff #4, Staff #5, and Staff #6, who did not meet lead teacher qualifications, were alone with a group of children.Plan of Correction: Staff #3 was covering during nap time, staff 4, 5, & 6 - the older kids have been combined and now with a degreed teacher.
Standard #: 8VAC20-780-340-F Description: Children under 10 years of age shall always be within actual sight and sound supervision of staff. During a 2-3 minute period, five children under age 8 were unsupervised while napping/resting in the multipurpose room, when staff exited the room to look for an attendance record. Plan of Correction: Staff was reminded to always request assistance if they need to leave the class.
