During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced inspection was initiated with a local agency on 9/16/2024 and completed on 10/30/2024 in response to a complaint received by the licensing office on 9/10/2024 relating to an allegation of a forbidden action. The inspector reviewed 1 child record and 1 staff record on site on 9/16/2024. This inspection included document review, interviews, observations and a tour of the facility.

The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/6/2024. A POC submitted after this date will not appear on the public website.

Description: The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Child #1, enrolled for over two years, did not have documentation of an immunization record on file.

Description: Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance. There was no physical examination in the record of child #1, enrolled for over two years.

Description: The center is required to obtain documentation of a negative tuberculosis (TB) screening at the time of employment and prior to coming into contact with children. Staff #1 has no screening documentation and has been employed for over one year.

Description: Each grouping of children shall have at least one staff regularly present who meets lead teacher qualifications. For two hours on 9/9/2024, staff #1, who did not meet lead teacher qualifications, was alone with a group of preschool aged children.

Description: The following actions are forbidden: physical punishment, striking a child, roughly handling a child. On 9/10/2024 staff #1 pinched child #1, age 3, leaving a mark on the child?s back, and causing the child to cry.
