An unannounced monitoring inspection was initiated on 08/26/24 and completed on 10/10/24 in response to a self-report received by the licensing office on 08/26/24 relating to behavioral guidance and supervision. The inspector reviewed one child?s record and one staff record on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/07/24. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-340-F Description: Children under 10 years of age always shall be within actual sight and sound supervision of staff.
On August 26, 2024, at approximately 8:00am, Staff #1 left nine, four-year-old, children out of sight and sound supervision for less than one minute to walk Child #1 into another classroom.Plan of Correction: Per the Center: "Ratios are a constant reminder daily and a refresher during
monthly staff meetings"
Standard #: 8VAC20-780-400-B Description: Behavioral guidance shall be constructive in nature, age and stage appropriate, and shall be intended to redirect children to appropriate behavior and resolve conflicts.
On August 26, 2024, Child #1 (4-years-old) was not following Staff #1's directions to say, "good morning" and to take their hat off when they were dropped off in the morning. Staff #1 pulled Child #1's hat off and walked him out of the classroom. While walking him into another classroom, Staff #1 was observed pulling Child #1 by the arm and saying: "Here. I am not dealing with him" and leaving him with another staff member and the child's parent.Plan of Correction: Per the Center: "Plans have been put in place to have ongoing training as needed and also refresh monthly during staff meetings.
The incident was self-reported and the employee has seperated from the company."
