During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Technical assistance was provided and standards reviewed in the following areas: 8VAC20-780-70 Staff records, 8VAC20-780-270 Building maintenance, 8VAC20-780-280 Hazardous substances and other harmful agents, 8VAC20-780-210 Program leader qualifications, background check protocol (as of July 1, 2024), and potable water testing requirements.
Effective January 1, 2025, the VDOE will begin determining compliance with ? 22.1-289.057 of the Code of Virginia, which is legislation passed by the General Assembly in 2020. The law requires all licensed child day programs, religious exempt child day centers that serve preschool age children, and certified preschools to test potable drinking water. The law requires that programs submit their plans and test results to the Virginia Department of Health Office of Drinking Water (VDH ODW) and the Superintendent. If the results of the testing indicate elevated lead levels, the program shall remediate, retest, and resubmit results to VDH ODW and the Superintendent. There is an additional alternative bottled water option that comes with additional requirements. An unannounced, on-site monitoring inspection was initiated and completed on 10/22/24. The on-site inspection began at 10:05am and ended at 1:05pm. The inspector reviewed compliance in the areas listed above. There were 10 children present and 10 staff. The inspector reviewed five children?s records and four staff records onsite. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/8/24. A POC submitted after this date will not appear on the public website.

Standard #: 22.1-289.011-F Description: The license and any other documents required by the Superintendent shall be posted in a
conspicuous place on the licensed premises.
The results of the most recent inspection were not posted, which is required. The inspection findings posted at the center were from December 2023.Plan of Correction: We will contact our licensing agent to confirm we have the latest inspection and will post it.
Standard #: 8VAC20-780-160-A Description: The documentation of a Tuberculosis screening (TB) shall have been completed within the last 30 calendar days of the date of employment.
The documentation in Staff #1?s record showed that the TB screening was not completed within 30 calendars days of the staff's date of employment.Plan of Correction: We have updated our onboarding to the 2023 standards, replacing the 2021 guidelines, and will consult our licensing inspector for any state updates.
Standard #: 8VAC20-780-70 Description: Staff records shall be kept for each staff person and contain all the required information.
The documentation for Staff #2?s reference checks did not contain dates of contact or the signature of the person making the call.Plan of Correction: We will instruct staff to write directly on the reference check form rather than on loose papers, which may be lost or misplaced outside the staff member's folder.
Standard #: 8VAC20-780-270-A Description: Areas and equipment of the center, outside, shall be maintained in a safe condition.
On the outdoor playground, there was a tree limb protruding approximately five inches from the ground, posing a tripping hazard.Plan of Correction: Staff were instructed to report any holes dug by the children that could pose a tripping hazard, such as the small tree stump. The hole has been filled, and the stump, measuring 2 inches, has been removed.
Standard #: 8VAC20-780-280-B Description: Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.
There was a bottle of sanitizer, left unlocked, in the three-to-four-year-old classroom. The sanitizer was accessible to children.Plan of Correction: We have informed staff that all cleaning agents should not be placed on the 6 foot dividers, even if out of children's reach or when no children are present. According to state guidelines, if it?s not securely stored, it is considered accessible. All cleaners must be locked away.
