During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. An unannounced, on-site focused inspection was initiated and completed on 10/28/2024, as a part of probation. The Department issued a Special Order on 12/22/2023 to place the center on probation for a period of 12 months as a result of high-risk violations. The on-site inspection began at 12:18 pm and ended at 2:07 pm. The inspector reviewed compliance in the areas listed above. There were 104 children present and 13 staff. The inspector reviewed 10 children?s records and 7 staff records on-site.
The inspection included document review, tour of the facility, interviews, and observations.
The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
