Comments:
An unannounced monitoring inspection was initiated on August 20, 2024 and completed on October 8, 2024 with a local agency in response to a self-report received by the licensing office on August 5,2024 relating to programs and staffing and supervision. The inspector reviewed 1 child record and 2 staff records electronically on September 24, 2024. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on November 13,2024. A POC submitted after this date will not appear on the public website.

Violations:
Standard #: 8VAC20-780-340-A
Description: When staff are supervising children, they shall always ensure their care, protection, and guidance.

On August 2, 2024, Child#1 (age 18 months ) was sitting at a table with other children, and was one of two children strapped into a toddler chair with a booster seat attached to it.

Child#1 pushed away from and tilted the table forward with his hands and Staff#1 pulled the chair the child was strapped in and pushed him to face a wall.

Child#1 used his feet to push away from the wall and Staff#1 picked the chair up with the child still strapped inside and carried him to another table across the room. Child#1 rocked the chair back and forth and attempted to exit multiple times before falling over with the chair still attached to his body sustaining an injury to his lip.

The booster seat, which was attached to the toddler chair, was only intended to be used with dining chairs.

Plan of Correction: On day of incident, an emergency staff meeting was held, in which the video of the incident was shown to all teachers present disciplinary write ups was written for the teachers in question. Reiterated the importance of sight/supervision as well as what the feeding booster chairs is for- feeding only. booster chairs is not a form of punishment.

Description: The following actions or threats thereof are forbidden: enclosure in any space that child cannot freely exit himself.

On August 2, 2024 Child#1 (age 18 months) was strapped into a toddler chair with a booster seat attachment for approximately 10 minutes with no activity.

The child rocked the chair back and forth and attempted to exit the chair multiple times before falling over onto the floor with the chair still attached to their body.

Plan of Correction: On day of incident, an emergency staff meeting was held, in which the video of the incident was shown to all teachers present. disciplinary write ups was written for the teachers in question. Standard #: 8VAC20-780-430-A
Description: Furnishings, equipment, and materials shall be of an appropriate size for the child using it.

The center strapped a booster seat onto a toddler chair for an 18 month old child. The booster seat is only to be used on dining chairs with a seat at least 14 inches wide, 14 inches deep, and a backrest at least 12 inches high.

Plan of Correction: As soon as the inspector advised me that the booster seat can not be used the toddler seats. They were removed that day. The seats are no longer used in that manner
