During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced, on-site renewal inspection was initiated on 11/01/2024 and completed on 11/01/2024. The on-site inspection began at 9:32am and ended at 12:10pm. The inspector reviewed compliance in the areas listed above. There were 23 children present and six staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included document review, tour of the facility, and interviews.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/21/2024. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-130-A Description: The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
Child #3, who has been attending for over two months, did not have documentation of an immunization record on file.Plan of Correction: Director contacted parent and informed parent immunization record was needed asap.
Standard #: 8VAC20-780-160-A Description: Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment.
Staff #1, who has been employed for one month, did not have a completed tuberculosis screening.Plan of Correction: Asst. Director will make sure all new staff members get TB screening before coming in contact of children.
Standard #: 8VAC20-780-160-C Description: A tuberculosis (TB) screening is required every two years from the last screening.
Staff #5, who is currently working, did not have a completed TB screening. The updated screening was due over one month ago.Plan of Correction: Staff has to complete updated TB screening.
Standard #: 8VAC20-780-180-A Description: Staff shall be of good character and reputation.
On 11/01/2024, a tuberculosis screening was observed in the record for staff #1 and staff #5
that had been altered. The director was shown the documentation and agreed that the documents appeared altered.Plan of Correction: Staff was spoken to by director and staff 1 & 5 were to obtain new TB documentation.
