During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Technical Assistance:
Effective January 1, 2025, the VDOE will begin determining compliance with ? 22.1-289.057 of the Code of Virginia, which is legislation passed by the General Assembly in 2020. The law requires all licensed child day programs, religious exempt child day centers that serve preschool age children, and certified preschools to test potable drinking water. The law requires that programs submit their plans and test results to the Virginia Department of Health Office of Drinking Water (VDH ODW) and the Superintendent. If the results of the testing indicate elevated lead levels, the program shall remediate, retest, and resubmit results to VDH ODW and the Superintendent. There is an additional alternative bottled water option that comes with additional requirements. Comments:
An unannounced, on-site monitoring inspection was initiated and completed on 11/6/2024. The on-site inspection began at 1:45pm and ended at 3:00pm. The inspector reviewed compliance in the areas listed above. There were 34 children present and 10 staff. The inspector reviewed 3
children?s records and 3 staff records on-site on 11/6/2024. This inspection included: document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.035-B-4
Description: Employees are required to authorize the child day center to obtain a copy of the results of a
criminal history record information check, a sex offender registry check, and a search of the
child abuse and neglect registry or equivalent registry from any state in which the individual
has resided in the preceding five years within 45 days of employment.

Staff #2 lived out-of-state in the preceding five years and did not have results of a criminal history record information check, a sex offender registry check and a search of the child abuse and neglect registry from the state in which the individual resided in the previous 5 years. Staff #2 had been employed for over 45 days.

Plan of Correction: I will submit the out-of-state background check requests right away.

Standard #: 8VAC20-780-160-A
Description: Each staff member and individual from an independent contractor shall submit
documentation of ta negative tuberculosis screening at the time of employment and
prior to coming into contact with children. The documentation shall have been completed
within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.

Staff #3 did not have a tuberculosis screening on file that was completed within the last 30
calendar days of their date of employment.

Plan of Correction: I will ask the staff to complete a new screening.
