During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the center was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced monitoring inspection was initiated on October 21, 2024 and completed on October 25, 2024, in response to a self-report received by the licensing office on October 17, 2024 relating to care and supervision of children. The inspector reviewed 1 child record and 2 staff records on-site. This inspection included document review interviews with staff, policies and procedures, emergency drill logs, medication administration logs, authorization forms. The investigation was re-opened on November 13, 2024 based on new information received by the licensing office from another agency. An additional review was initiated on November 14, 2024 and completed November 26, 2024. The on-site inspection began at 11:00am and ended at 12:15pm. The inspection included review of additional staff file, interviews with staff, medication forms, viewing of closed-circuit video footage from October 17, 2024.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 63.2-1509-A Description: Staff employed by a child day program who have reason to suspect that a child has been hurt through the actions of an adult will report the matter immediately to the local department's toll-free child abuse hotline.
Staff were aware of the burn incident, and no one from the center reported it to CPS.Plan of Correction: Will refresh training for all staff on the importance of timely mandated reporting when child abuse is suspected or known, including at the center.
Standard #: 8VAC20-780-270-A Description: The center will ensure that all areas of the center are maintained in a safe condition.
Child A was burned by hot coffee brought into the classroom and left in an area accessible to
children. Child A received medical care from a physician.Plan of Correction: Will refresh training for all staff on the importance of maintaining a safe environment for the children attending the center.
Standard #: 8VAC20-780-340-A Description: When staff are supervising children, they will always ensure their care, protection, and
guidance.
1. On 10/17/2024, Child A (age 28 months) knocked over a mug of hot coffee that was placed, by a staff member, on a surface within reach of children.
2. Staff did not immediately perform first aid for Child A although blisters started to form on child's arm and chest.Plan of Correction: Will refresh training for all staff on preventative safety strategies when supervising, caring for, and safeguarding children at the center.
Standard #: 8VAC20-780-520-C Description: If ointment or cream is used, the following requirements will be met:
1. Written parent authorization will be obtained;
2. These products shall be in the original container and labeled with the child's name;
3. A record will be kept that includes the child's name, date of use, frequency of application and any adverse reactions.
As noted on the incident report and confirmed with the director, the staff applied Vaseline ointment to the child's blisters 20 minutes after the burns occurred. There was no written permission given for the treatment by the parent.Plan of Correction: Must not have expired and parent-signed "Written Medication Consent Form" is on file (Per Hopkins House Policy #24 ("Emergency Care & Sick Child")
Will refresh training for all staff on Hopkins House #24 ("Emergency Care & Sick Child"). This policy outlines proper labeling, recording, reporting on use of topical ointments and medications, and lists the types of permissible ointments and medications that may be used at the center, when and under what conditions.
Will also refresh training for all staff on Hopkins House Policy #38 ("Young Scholar Medication"). This policy requires that only MAT certified staff members are permitted to administer medication to children.
Standard #: 8VAC20-780-550-N Description: The center will notify the parent immediately when a child sustains a serious injury.
The parent was not notified of the child's injury until 20 minutes after the serious injury occurred.Plan of Correction: Will refresh training for all staff on the importance of timely notification to parents when their child experiences an injury at the center.
