An unannounced monitoring inspection was conducted in response to a self-report received by the licensing office on 10/28/24 relating to supervision and administration. The inspector reviewed 1 child?s record and 2 staff records on-site. This inspection included document review,
tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/23/24. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-40-E Description: REPEAT VIOLATION
The center must ensure that the center?s activities are maintained in compliance with its own policies as required by the standards.
The center?s supervision policy states that ?Staff should be aware of how many children are in their care and where all children are, at all times. The policy also states, ?Supervision of children (indoor and outdoor) - Know the names and the number of children in their care at all times, use transition tracking to account for the children in their care, conduct and record transitions with face to name roll call, conduct a head count whenever a transition through a door or gate occurs, conduct periodic head counts throughout the day, in addition to those required during transitions, log in and out whenever they join or leave a group.
On 10/23/24 at approximately 6:18pm, three-year old Child # 1 was left alone and unsupervised on the playground for approximately two minutes. Staff # 1 and Staff # 2 were responsible for supervising a group of children on the playground and gathered the children to come inside at 6:18 pm. Both staff stated that they did not count the children and did not complete a face to name count, as per the center?s supervision policy.Plan of Correction: We have reviewed the supervision policy, retrained the staff on monitoring attendance during transitions.
Standard #: 8VAC20-780-340-F Description: Children under 10 years of age shall always be within actual sight and sound supervision of staff.
On 10/23/24, at approximately 6:18 pm, three year-old Child # 1 was left alone, and out of sight and sound supervision on the playground for approximately two minutes.
Staff # 1 and Staff # 2 were the staff members responsible for a group of five three- to four-year-old children. The children were taken to the playground at approximately 5pm.
At approximately 6:18 pm, Staff # 1 and staff # 2 lined up the children and brought them inside, unaware that they left Child #1 behind.
Witness # 1 stated that she arrived to the center at approximately 6:20pm to pick up her child. While standing at the entrance door, Witness # 1 heard a child crying. When Witness # 1 looked over at the playground fence, she saw Child # 1 standing there, crying. Witness # 1 immediately went inside and notified Staff # 3 who was in the hallway and went to get Child # 1. Witness # 1 stated that when she took Child # 1 to the classroom, Staff # 1 and Staff # 2 looked surprised to see them.Plan of Correction: The teacher was terminated for failing to adhere to the supervision policy. We have retrained all employees on the supervision policy to ensure compliance and consistency. The administration is closely monitoring all face-to-name interactions and overseeing supervision in every classroom.
