An unannounced, on-site monitoring inspection was initiated on 11/14/2024 and completed on 11/14/2024, as a part of the licensure period. The on-site inspection began at 9:28AM and ended at 10:37AM. The inspector reviewed compliance in the areas listed above. There were 13 children present and 5 staff. The inspector reviewed 3 children?s records on-site 11/142024 and 2 staff records 11/14/2024. This inspection included - document review (i.e. Injury logs, policies and procedures, emergency drill logs, medication administration logs, authorization forms), tour of the facility, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 8VAC20-780-150-B Description: Each report shall be signed by a physician, his designee, or an official of the local health department. The record for Child #1, enrolled 1 day, did not have an immunization record signed by a physician. Plan of Correction: Provider will obtain a signed immunization record.
Standard #: 8VAC20-780-60-A Description: Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The records for Child #1, enrolled 1 day, was missing the doctors name. Plan of Correction: Provider will obtain doctors name.
