Comments:
An unannounced monitoring inspection was initiated and completed on 10/24/2024 with a local CPS agency in response to a self-report received by the licensing office on 10/22/24 relating to Staffing and Supervision. The inspector reviewed 4 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
The Department requested that the provider complete an IPOC as result of this inspection related to Supervision of Children, Operational Responsibilities and Attendance Records.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/27/24. A POC submitted after this date will not appear on the public website.

Violations:
Standard #: 8VAC20-780-40-E
Description: The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with the center's own policies and procedures.

The center?s policy/protocol regarding knowing where children are verbally and written tracking/name-to-face forms, for the Cats classroom from October 15-18th and 21st, 2024, were not accurate.

Plan of Correction: School Leader met with regional director to discuss center policies and procedures to maintain compliance with licensing and state law/ standards.
Training for all staff on Name to face sheet will be completed at all staff meeting on 11/21/24.

Staff will complete Name to Face sheet during transitions to and from the classroom. When going outdoors, staff must complete Name to face in the classroom before exiting the building and before entering the playground, in additon to a child head count. Closing and opening admin, verfiies and check Name to Face sheets daily, as well as checks during tranistion times to ensure procedure is done correctly.
Procedure for tranisitoning children to another classroom or teacher was clarified.

Standard #: 8VAC20-780-80-A
Description: For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.

A review of written record of arrival and departure of children was incorrect on 10/21/24 at 5:20pm. Documented time of last child departing was 5:20pm. 5 student departures were unaccounted for; nine children on record as present, but only four were present.

Plan of Correction: Immediately, a written attendance sheet was created for all classrooms to account for children's arrival and departure from the center.

Description: Children under 10 years of age always shall be within actual sight and sound supervision of staff.

On 10/21/24 Staff left a 3yo alone on the enclosed school playground from approximately 5:35 pm to 5:43 pm.

Plan of Correction: Immediately, staff have been trained on written Name-to Face sheets. School leader met with Regional Director to discuss plans of action on Staff complete name to face during transition to and from the classroom. When going outdoors, staff must complete Name to face in the classroom before exiting the building and before enterning the playground, in additon to a child head count.
Importance of supervision and safety was communicated in 3 ways (verbally, written in staff newsletter and training). All staff will complete Active Supervision training on RISE by Dec 6th.
