During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed.

~Water Lead Testing: If a licensed child day program wishes to be added to the VDH waitlist for the grant program, please contact leadtestingprogram@vdh.virginia.gov. Providers on the VDH grant program waitlist and those who are enrolled but have not yet completed testing, will be in compliance as long as they continue to participate in the VDH program and are following their plan. Enrolled programs should have evidence of enrollment and be able to demonstrate enrollment via the 120 Water portal. VDOE will confirm a program?s status on the waitlist to determine compliance. The waitlist is a courtesy and not a guarantee that a program will receive funding. Providers must take action
to join the program when notified about the opportunity for enrollment. A provider may remain on the wait list for three months; however, if the provider has not enrolled in the grant program or if the grant program has reached capacity at the end of the three-month waiting period, the provider must continue to take action toward compliance by completing testing with another approved laboratory.

An unannounced, on-site renewal inspection was initiated on November 18, 2024 and completed on November 18, 2024, as a part of the current licensure period. The on-site inspection began at 9:45am and ended at 11:30am. The inspector reviewed compliance in the areas listed above.
There were 117 children present and 17 staff. The inspector reviewed 10 children?s records and 5 staff records on-site. This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 8VAC20-780-150-B Description: Each report will include the date of the physical examination and dates immunizations were received and shall be signed by a physician, his designee, or an official of a local health department.
Child A had immunizations on file that are not signed by the physician.Plan of Correction: Documentation will be signed by physician's office.
Standard #: 8VAC20-780-60-A Description: Each child's record will contain name, home address, home phone number, and work phone number of each parent, and the name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached.
Child A's file does not have a second emergency contact listed. Child B's file contains both parents as emergency contacts. Child G has no home information or work phone number listed for second parent.Plan of Correction: Information will be obtained from parents.
