Inspector reviewed the following with the provider potable water lead testing January 2025; updated background check; administration-children's record completion; 350 staffing and supervision, 450-linens; 270 building maintenance-200 Program director shall regularly be on site at least 50% of the center's hours of operations.

n unannounced, on-site monitoring inspection was initiated on November 14, 2024 and
completed on November 21, 2024 as a part of the licensure period. The on-site inspection began at 10:14 am and ended at 11:41 am. The inspector reviewed compliance in the areas listed above. There were 20 children present and 5 staff. The inspector reviewed 5 children's records
and 3 staff records electronically on November 19, 2024. This inspection included document review, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or
law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected.
