Technical Assistance:

1. Standard 240 Orientation Training was reviewed with the Directors. 2. when accepting immunization records, be sure to check that the physician verified the child has been "adequately immunized. 3. Effective January 1, 2025, VDOE will begin determining compliance with ? 22.1-289.057 of the Code of Virginia, which is legislation passed by the General Assembly in 2020. The law requires all licensed child day programs, religious exempt child day centers that serve preschool age children, and certified preschools to test potable drinking water. Comments:

An unannounced focused inspection was completed on 11/20/2024 as a part of the conditional licensure period. The on-site inspection began at 2:00pm and ended at 3:30pm. The inspector reviewed compliance in the areas listed above. This inspection included: document review, tour of the facility, interviews, observations and measurements. There were 2 children present and 3 staff. The inspector reviewed 2 children?s records and 2 staff records. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:

Standard #:	22.1-289.035-B-1
Description:	Staff are required to provide a sworn statement or affirmation that includes whether the staff has lived outside the state of Virginia within the last 5 year. Staff #1 and #2 did not have a sworn statement disclosing if they have lived outside the state of Virginia within that last 5 years.
Plan of Correction:	An old version of the sworn statement was given to the staff instead of the latest version that includes the question about if staff have lived outside the state within the last 5 years. The new statements were printed and given to the staff to complete and sign.
Standard #:	8VAC20-780-70
Description:	Each staff record is required to maintain documentation to demonstrate that the individual possesses the education, certification, and experience required by the job position, and orientation and training as required in 8VAC20-780-240 and 8VAC20-780-245. Staff #1 and #2 did not have documentation of having completed an orientation that includes all the required trainings.
Plan of Correction:	The directors will ensure that each staff member receives an orientation that covers all required information. A checklist will be kept for each staff showing how they demonstrate compliance.
Standard #:	8VAC20-780-450-A
Description:	When cots and mats are used by children during the designated rest period, they shall have linens consisting of a top cover and a bottom cover. A child was observed sleeping on a mat that did not have a top or bottom cover.
Plan of Correction:	the directors will ensure all cots and mats children use during rest time have a top and bottom cover.
