During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
- Technical Assistance:

Effective January 1, 2025, the VDOE will begin determining compliance with ? 22.1-289.057 of the Code of Virginia, which is legislation passed by the General Assembly in 2020. The law requires all licensed child day programs, religious exempt child day centers that serve preschool age children, and certified preschools to test potable drinking water. The law requires that programs submit their plans and test results to the Virginia Department of Health Office of Drinking Water (VDH ODW) and the Superintendent. If the results of the testing indicate elevated lead levels, the program shall remediate, retest, and resubmit results to VDH ODW and the Superintendent. There is an additional alternative bottled water option that comes with additional requirements. An unannounced, on-site monitoring inspection was initiated on 11/18/24. The inspection began at 12:35pm and ended at 3:20pm. The inspector reviewed compliance in the areas listed above. There were 30 children present and 7 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, observations, and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 12/2/24. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-50-A Description: The Center is required to treat that staff and child records confidentially.
Evidence: There were records posted in the lobby at the center dated 6/4/24 that contained confidential information related to the staff.Plan of Correction: Going forward, we will implement strict protocols to ensure that all staff and child records are securely stored and only accessible to authorized personnel, with regular staff training on confidentiality policies to prevent future violations. Standard #: 8VAC20-780-60-A-8 Description: The Center is required to ensure they have a written care plan for each child with a diagnosed food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.
Evidence: The record for Child #1, Child #2, and Child #3 did not contain an allergy care plan for their diagnosed food allergy.Plan of Correction: All management will receive a training on LCG's Child Allergy Procedure 12/5/2024
Standard #: 8VAC20-780-70 Description: (Repeat Violation) The Center is required to maintain documentation for each staff member that includes documentation that two or more references as to character and reputation as well as competency were checked before employment.
Evidence:
The center did not have documentation for Staff #1 that two or more references as to character and reputation as well as competency were checked before employment.Plan of Correction: To address the repeat violation, we will immediately obtain and document two reference checks for Staff #1, revise our hiring process to ensure reference checks are consistently completed before employment, and implement a monthly audit and tracking system to ensure ongoing compliance.
We will ensure all employee and child paperwork is completed and filed before their first day, and implement a system for regular audits and accountability. This includes creating checklists, providing training, utilizing digital tracking, and holding managers responsible for ensuring compliance at every step of the onboarding process.
Standard #: 8VAC20-780-280-B Description: (Repeat Violation) The Center is required to keep hazardous substances in a locked place using a safe locking method that prevents access by children.
Evidence:
1)During inspection in the unlocked kitchen directly off the Preschool classroom and where children have access, 1 container of sanitizer and 10 detergents containing a label that stated ?Keep out of Reach of Children? and the word ?Warning? were stored on a shelf.
2)In the Toddler classroom in an unlocked drawer was 1 container of baby powder and 2 containers of hand sanitizer containing a label that stated ?Keep out of Reach of Children? and the word ?Warning? on the label.Plan of Correction: Staff will be coached during team meeting scheduled on 12/9/2024, to promptly report any facility issues, such as broken locks, doors, and cabinets, to management. Additionally, staff will be trained on-site to remove hazardous items from compromised drawers and cabinets until repairs are completed.
Standard #: 8VAC20-780-420-E-3 Description: The Center is required to ensure annually that parent confirmation of the required information in the child record is up to date.
Evidence: The files for Child #1, Child #2, and Child #4 did not have documentation that the parent had confirmed the updated record annually.Plan of Correction: Management has a scheduled a file review on 12/20/2024. Moving forward management will calendar a file review every year in August.
Standard #: 8VAC20-780-510-E Description: The Center is required to have written medication authorization from the child?s physician for a long term medication.
Evidence:
For a long term medication for Child #3, there was no medication authorization from the physician.Plan of Correction: The Center will ensure that written medication authorization from the child's physician is obtained and maintained for all long-term medications, including for Child #3, moving forward. Standard #: 8VAC20-780-520-C Description: (Repeat Violation) The Center is required to ensure that diaper cream is kept inaccessible to children.
Evidence: In the Toddler classroom, there were 4 diaper creams stored in an unlocked drawer where children have access.Plan of Correction: Staff will be coached during team meeting scheduled on 12/9/2024 to promptly report any facility issues, such as broken locks, doors, and cabinets, to management. Additionally, staff will be trained on-site to remove hazardous items from compromised drawers and cabinets until repairs are completed.
Standard #: 8VAC20-780-550-D Description: (Repeat Violation) The Center is required to implement a monthly practice evacuation drill.
Evidence: There was no documentation of an evacuation drill for October 2024.Plan of Correction: October 2024 evacuation drill documentation has been addressed, and corrective measures have been implemented to ensure future drills are properly conducted and documented in compliance with the monthly requirement. Going forward we will put in our calendar to update this monthly. All staff will be trained on LCG's Emergency Drill Procedure 12/9/2024
