Comments:

An unannounced, on-site monitoring inspection was initiated on 11/20/2024 and completed on 11/26/2024, as a part of the licensure period. The on-site inspection began at 3:02PM and ended at 4:05PM. The inspector reviewed compliance in the areas listed above. There were 12 children present and 2 staff. The inspector reviewed 5 children?s records on-site 1/20/2024 and 2 staff records electronically 11/26/2024. This inspection included - document review, tour of the facility, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:

Standard #:	8VAC20-770-60-C-2
Description:	The center must request a search of the central registry within 30 days of the employee's first day of employment. Staff #1, who had been employed for twenty two months, did not have a central registry search requested.
Plan of Correction:	Provider will ensure backgrounds are complete.
Standard #:	8VAC20-780-60-A
Description:	Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The record for Child #1and Child #3, enrolled 3 months, were missing 7 required components. Child #2 and Child #4, enrolled 3 months, were missing 13 required components. Child #5, enrolled 3 months, was missing 9 required components.
Plan of Correction:	Provider will ensure childrens records are accessible.
Standard #:	8VAC20-780-70
Description:	The following staff records shall be kept for each person. The record for Staff #1, employed for 22 months, did not contain background checks as required; documentation to demonstrate that the individual possesses the education, certification, and experience required by the job position.
Plan of Correction:	Provider will ensure staff records are complete.
Standard #:	8VAC20-780-530-A
Description:	At least one staff in each classroom or area where children are present shall have within 90 days of October 13, 2021, current certification in cardiopulmonary resuscitation as appropriate to the age of the children in care and current certification in first aid. Staff #1, employed for 12 months, did not have CPR and first aid training appropriate to the age of the children in care.
Plan of Correction:	Provider will ensure staff receive appropriate CPR and first aid training.
Standard #:	8VAC20-780-560-J
Description:	Tables shall be sanitized after each use for feeding. A group of 12 children were observed seated at two tables during homework time that were not sanitized by staff after snack time.
Plan of Correction:	Provider will ensure tables are cleaned and sanitized after snack.
