During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Technical Assistance:
A review of center?s safe sleeping practices determined that their safe sleep policy is not consistent with the American Academy of Pediatrics Updated 2022 Recommendations for Reducing Infant Deaths in the Sleep Environment.
Effective January 1, 2025, the VDOE will begin determining compliance with ? 22.1-289.057 of the Code of Virginia, which is legislation passed by the General Assembly in 2020. The law requires all licensed child day programs, religious exempt child day centers that serve preschool age children, and certified preschools to test potable drinking water. The law requires that programs submit their plans and test results to the Virginia Department of Health Office of Drinking Water (VDH ODW) and the Superintendent. If the results of the testing indicate elevated lead levels, the program shall remediate, retest, and resubmit results to VDH ODW and the Superintendent. There is an additional alternative bottled water option that comes with additional requirements. The inspection findings were updated on April 22, 2025?
An unannounced, on-site monitoring inspection was initiated on November 21, 2024 and completed on November 21, 2024. The on-site inspection began at 10:10 a.m. and ended at 1:30 p.m.. The inspector reviewed compliance in the areas listed above. There were 87 children present and 18 staff. The inspector reviewed eight children?s records and eight staff records on-site & electronically on November 22, 2024. This inspection included documentation review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on December 12, 2024. A POC submitted after this date will not appear on the public website.

Violations:
Standard #: 8VAC20-780-40-E
Description: The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with these standards, the center's own policies and procedures that are required by these standards, and the terms of the current license issued by the department.
The center did not follow their own written procedures for safe sleeping practices which state the following: No loose bedding, pillows, bumper pads, etc. will be used in cribs.
In the Angelfish classroom a loose blanket was in the crib with a five-month-old sleeping baby.

Plan of Correction: The Angelfish teacher received disciplinary action and a plan of personal improvement. Immediate termination will happen moving forward for any employee. The entire staff received additional in depth sleep practice training verbally and in writing.

Standard #: 8VAC20-780-40-M
Description: The center is required to keep a current list of all known allergies, sensitivities, and dietary restrictions in each room or area where children are present. The list must be dated and kept confidential.
The allergy list in the Butterfly classroom was not accurate per staff. Two children were listed that no longer should be and one was not listed.

Plan of Correction: All lists were reviewed and updated with all restrictions included on all lists.

Standard #: 8VAC20-780-440-B
Description: Cribs, cots, rest mats, and beds shall be identified for use by a specific child.
In the Angelfish classroom one crib was not labeled with a child's name.

Plan of Correction: Starting to use a stronger tape to label cribs.

Standard #: 8VAC20-780-530-A-2
Description: At least one staff in each classroom or area where children are present shall have current certification in both CPR and first aid. Staff #6 was alone with a group of five, two-year-old children and was not certified in first aid.

Plan of Correction: Will double check all hires to ensure CPR & first aid certified not just CPR. CPR and first aid class scheduled for 12/5/24.

Standard #: 8VAC20-780-570-E
Description: Prepared infant formula shall be refrigerated, dated and labeled with the child's name.
In the Angelfish classroom two bottles did not have dates.

Plan of Correction: Both parent and teacher were reminded in writing of this policy.
