An unannounced, on-site monitoring inspection was initiated on 12/3/2024 and completed on 12/3/2024, as a part of the licensure period. The on-site inspection began at 10:23AM and ended at 12:23PM. The inspector reviewed compliance in the areas listed above. There were 11 children present and 5 staff. The inspector reviewed 4 children?s records on site on 12/3/2024 and 2 staff records on-site on 12/3/2024. Medication was reviewed on site 12/3/2024.
This inspection included - document review (i.e. Injury logs, policies and procedures, emergency drill logs, medication administration logs, authorization forms), tour of the facility, and observations (this would include observations of photos, recordings, etc.).
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 8VAC20-780-500-B Description: The diapering area shall have a nonabsorbent surface for diapering or changing. The diapering surface in the infant room was torn in two spots on the top. Plan of Correction: Provider replaced diaper changing pad.
