During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Effective January 1, 2025, the VDOE will begin determining compliance with ? 22.1-289.057 of the Code of Virginia, which is legislation passed by the General Assembly in 2020. The law requires all licensed child day programs, religious exempt child day centers that serve preschool age children, and certified preschools to test potable drinking water. The law requires that programs submit their plans and test results to the Virginia Department of Health Office of Drinking Water (VDH ODW) and the Superintendent. If the results of the testing indicate elevated lead levels, the program shall remediate, retest, and resubmit results to VDH ODW and the Superintendent. There is an additional alternative bottled water option that comes with additional requirements. An unannounced, on-site monitoring inspection was initiated on 12-2-2024 and completed on 12-2-2024. The on-site inspection began at 10:40am and ended at 1:00pm. The inspector reviewed compliance in the areas listed above. There were 37 children present and 10 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, observations and measurements.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five 5 business days from today, which will be the close of business on12-11-2024. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-140-A Description: Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance. Standard #: 8VAC20-780-40-M Description: The center shall maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions documented in the allergy plan. This list shall be dated and kept confidential in each room or area where children are present.
1.The allergy list in room #2 was posted in a way that people could see it and was not kept confidential.

Standard #: 8VAC20-780-60-A Description: Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information.
1.The record for Child #1 did not have the name of the second parent listed or the appropriate legal paperwork shall be on file when the custodial parent requests the center not to release the child to the other parent.
2. The record for Child #5 did not have a written care plan as required for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction. Standard #: 8VAC20-780-70 Description: The provider shall keep for each staff person the following information for each staff record. Staff #5 who has been working for the past 4 months and was missing documentation of information related to any health issues that may interfere with the job and documentation to demonstrate that the individual possesses the education required by the job position. Standard #: 8VAC20-780-270-A Description: Areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition. On the playground there two seats on the merry-go-cycle that were cracked and broken causing pinch points. Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: 8VAC20-780-330-B Description: Playground equipment shall comply with minimum safety standards and resilient surfacing shall be under equipment with moving parts or climbing apparatus to create a fall zone free of hazardous obstacles. Fall zones are defined as the area underneath and surrounding equipment that requires a resilient surface. A fall zone shall encompass sufficient area to include the child's trajectory in the event of a fall while the equipment is in use. Falls zones shall not include barriers for resilient surfacing.
1.The fall zone for the blue slide chute did not have the required 6 feet of fall zone because there was a picnic table approximately 3 feet 6 inches from the slide chute.

Standard #: 8VAC20-780-340-F Description: Children under 10 years of age shall always be within actual sight and sound supervision of staff except that staff need only be able to hear a child who is using the restroom. Child #3, age 3 was observed without sound supervision in the centers hallway restroom as Child #3 was alone and no other staff were in the hallway and all of the centers classrooms doors were closed.
