An unannounced, on-site focused inspection was initiated on 11/20/2024 and completed on 11/20/2024, as a part of the provisional licensure period. The on-site inspection began at 10:00am and ended at 12:45pm. The inspector reviewed compliance in the areas listed above. There were 128 children present and 20 staff. The inspector reviewed eight staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:

Standard #:	8VAC20-780-70
Description:	Documentation of at least 2 references to character, reputation, and competency are to be checked prior to employment, if a reference is taken over the telephone it shall include the required information. The two references in the record of Staff #2 were checked after the staff started employment. One reference was checked two days after employment and the second reference was checked 18 days after employment. Two references for Staff #5 , who has been actively working at the center for the past three months, were checked over the telephone, but the documentation did not include the signature of the person making the call. Staff #7, who has been actively working at the center for two days, did not have any completed reference verifications on file.
Plan of Correction:	Moving forward, the center will make sure reference checks are received before employment.
