During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced, on-site monitoring inspection was initiated and completed on November 22, 2024 as a part of probation. The Department issued a Special Order on June 25, 2024 to place the licensee on probation for a period of twelve (12) months. The Department issued a Notice of Intent to deny renewal application for a license on November 13, 2024 as a result of high-risk violations.
The on-site inspection began at 11:00 a.m. and ended at 3:30 p.m. The inspector reviewed compliance in the areas listed above. There were 53 children present and 8 staff. The inspector reviewed four children's records electronically and on staff record on-site on November 12, 2024. This inspection included document review, tour of the facility and interviews.
Information gathered during the inspection determined non-compliance with applicable standards or law, and a violation was documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for the violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on December 17, 2024. A POC submitted after this date will not appear on the public website.

SYSTEMIC VIOLATION
Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.
1. On the left side of the infant room, there was a cracked and splintered wood frame around an access door that creates a scratch and pinching hazard. Underneath of the access panel, there was chipped and peeling paint around the baseboards in multiple areas.Plan of Correction: Not available online. Contact Inspector for more information.
