During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Effective January 1, 2025, the VDOE will begin determining compliance with ? 22.1-289.057 of the Code of Virginia, which is legislation passed by the General Assembly in 2020. The law requires all licensed child day programs, religious exempt child day centers that serve preschool age children, and certified preschools to test potable drinking water. The law requires that programs submit their plans and test results to the Virginia Department of Health Office of Drinking Water (VDH ODW) and the Superintendent. If the results of the testing indicate elevated lead levels, the program shall remediate, retest, and resubmit results to VDH ODW and the Superintendent. There is an additional alternative bottled water option that comes with additional requirements. An unannounced, on-site monitoring inspection was initiated and completed on 11/4/2024. The on-site inspection began at 9:45am and ended at 11:30am. The inspector reviewed compliance in the areas listed above. There were 15 children present and 3 staff. The inspector reviewed 5 children?s records and 3 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 12/19/2024. A POC submitted after this date will not appear on the public website.

Standard #: 22.1-289.035-B-2 Description: The center must obtain national criminal background check results prior to the employee's first day of employment.
Staff #3 was employed for over three weeks before the center obtained fingerprinting results.Plan of Correction: Will review files before a new employee starts.
Standard #: 22.1-289.035-B-3 Description: The center must request a search of the central registry prior to the employee's first day of employment.
Staff #1, who has been employed for over three weeks, did not have a central registry search requested. Staff #1 was working alone with children.Plan of Correction: CPS search will be started when offer letter is accepted.
Standard #: 8VAC20-780-160-A Description: Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment.
Staff #1 has been employed for over four months and did not have documentation of a negative TB screening on file.Plan of Correction: This has been corrected. Will review files before starting new staff.
Standard #: 8VAC20-780-70 Description: Documentation of at least two references to character, reputation, and competency are to be checked prior to employment.
Staff #3, has been employed for over three weeks, did not have any completed reference verifications on file.Plan of Correction: We have obtained these. Will review all files before staff starts work.
Standard #: 8VAC20-780-240-A Description: Staff are to complete the Virginia Department of Education sponsored orientation course within 90 days of employment.
Staff #1 has been employed for four months and has not yet completed the Virginia Department of Education sponsored orientation course.Plan of Correction: Staff person is working on this. We will add completion dates for new hires to our calendars.
Standard #: 8VAC20-780-270-A Description: Areas and equipment inside the center shall be maintained in a clean, safe, and operable condition.
In the Tech room, a small utility door in the lower wall was open and unable to be locked. Inside the utility space were piles of chipped paint and dirt. In addition, in the downstairs girls bathroom, there was a sticky pest trap on the floor with multiple insects on it. Finally, in the upstairs men's restroom, tiling was missing in the lower wall area exposing rust and crumbling drywall.Plan of Correction: Facilities team has been notified and started correcting problems.
Standard #: 8VAC20-780-510-P Description: When an authorization for medication expires, the facility shall notify parents that the medication must be picked up within 14 days of the authorization expiring or that a new authorization should be completed.
Child #3 had prescription medication on site, but the parental medication authorization expired over 60 days ago. The authorization had not been renewed and the medication was not returned or disposed of as required.Plan of Correction: Will add all expiration dates to calendar to include management. Prescription returned to caregiver and will get a new form.
Standard #: 8VAC20-780-530-A Description: At least one staff in each classroom or area where children are present shall have current certification in both CPR and first aid.
Staff #1 was alone with a group of nine children and does not have currently CPR/First Aid certification.Plan of Correction: We have scheduled everyone who needs it for training on Jan.11. Until then they will not be alone with kids. Staff will work in teams until they are certified.
Standard #: 8VAC20-780-550-H Description: The center shall maintain a record of the dates of required practice drills for one year.
There was no documentation of the two required shelter-in-place drills or the annual lockdown drill for the past year.Plan of Correction: Drill dates will be added to calendars and recorded monthly.
