An unannounced, on-site monitoring inspection was initiated on 12/05/2024 and the inspection concluded on 12/11/2024. The on-site inspection began at 4:30 p.m. and ended at 6:00 p.m. The inspector reviewed compliance in the areas listed above. There were 30 children present and 4 staff. The inspector reviewed 6 children?s records on site and 3 staff records electronically on 12/11/2024. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 8VAC20-780-60-A Description: The facility shall have a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction. Child A has diagnosed food allergies but did not have a written allergy action plan on file. Plan of Correction: Staff will request an allergy action plan for Child A.
Standard #: 8VAC20-780-240-A Description: Staff shall complete the Virginia Department of Education-sponsored orientation course (VA Pre-service) within 90 days of employment. Staff 3 (employed for 11 months) did not have documentation of having completed the VA Dept of Education sponsored orientation course within 90 days of employment. Plan of Correction: Staff will take the Va Preservice training.
Standard #: 8VAC20-780-540-E Description: The following nonmedical emergency supplies shall be required: one working, battery-operated radio in each building used by children. The radio on site was a crank radio without batteries. Plan of Correction: Staff will get a battery operated radio for the emergency kit.
