During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced, on-site monitoring inspection was initiated and completed 12/9/2024 The on-site inspection began at 2:45 pm and ended at 4:25 pm. The inspector reviewed compliance in the areas listed above. There were 108 children present and six staff. The inspector reviewed five staff records on-site and electronically on 12/12/24.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 12/30/2024. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-160-A Description: REPEAT VIOLATION
Each staff member shall submit documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with the children.
The screening for Staff #5 has not been completed. Staff #5 has been actively working at the center for eight months.Plan of Correction: At BGCME Abingdon, TB screenings will be required prior to working with children.
Standard #: 8VAC20-780-70 Description: REPEAT VIOLATION
Documentation of at least 2 references to character, reputation, and competency are to be checked prior to employment.
Staff #1-5, who has been actively working at the center for the past 7 months, did not have any completed reference verifications on file.Plan of Correction: The references for the staff have been completed. 6/12/24-10/1/24.
Standard #: 8VAC20-780-240-B Description: REPEAT VIOLATION
Staff shall complete orientation training in subsection C of this section prior to the staff member working alone with children and no later than seven days of the date of assuming job responsibilities.
Staff # 1-5, who have been actively working at the center for eight months, do not have documentation on file of completing orientation training.Plan of Correction: The Unit Director will meet individually with each staff member to review the orientation training.
Standard #: 8VAC20-780-350-B-5 Description: The required staff-to-children ratio for school age eligible to 9 years old, with the ratio waiver, is 1:20.
Three staff were observed supervising 98 children from 3:00 pm to 3:24 pm.Plan of Correction: We are currently in the process of hiring new staff.
Standard #: 8VAC20-780-550-D Description: The center shall implement a monthly practice evacuation drill.
There was no documentation of an evacuation drill for the month of September, 2024.Plan of Correction: The Unit Director will ensure that all drills are documented each month.
