During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. An unannounced, on-site monitoring inspection was initiated on 12/03/2024 and completed on 12/12/2024. The on-site inspection began at 10:00am and ended at 12:45pm. The inspector reviewed compliance in the areas listed above. There were 55 children present and 12 staff. The inspector reviewed six children?s records and six staff records electronically on 12/05/2024 and on 12/13/2024. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 12/23/2024. A POC submitted after this date will not appear on the public website.

Standard #: 22.1-289.035-A Description: The center is required to have employees undergo a background check every five years. The record for Staff #3 contained a completed national criminal background registry check that was overdue by a one year and four months. Standard #: 8VAC20-780-70 Description: Staff records shall be kept for each staff person with all of the required information. The record for Staff #3 did not contain documentation to demonstrate that the individual possesses the education and training. Staff #3 has worked at the center for over 14 years. The record for Staff #4 did not contain documentation to demonstrate that the individual possesses the education and training. Staff #4 has worked at the center for over 1 year 10 months. Standard #: 8VAC20-780-280-B Description: Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children. Standard #: 8VAC20-780-500-B Description: Disposable diapers shall be disposed in a leakproof or plastic-lined storage system that is either foot-operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal.
